APPLICATION INSTRUCTIONS
- Prepare your team information (Name, Email, Role) and identify a team leader who must be a member of team.
- Obtain the superintendents signed Letter of Intent and save as a PDF to upload. See Template
- Answer the application short response questions.
- Complete the Application form by December 2, 2025
The superintendent and team leader will be notified by December 5th if their team has been selected to be part of the 2026 PEAK Cohort.
